PARTICIPANT AGREEMENT & RELEASE

In consideration of the services of Core24 Gym, I hereby agree to release and discharge Core24 from any and all liability.

1. I acknowledge that exercise entails inherent risks of physical injury.

2. I voluntarily assume all risks associated with my participation.
